"fixed disk controller failure" is something you normally get with IDE when the CHS parameters are incorrect. Are you sure you have entered them correctly in the BIOS? Should be 958/12/34. also, make sure the jumpers on the card are set correctly and the IDE is enabled, if not will display this …

oouch, i have a similar mobo, an octek fox II, you could remove the sticker so we can read whats behind it. my guess its a programable array logic chip, kinda difficult to replace because they need to be burnt like an eeprom.

quake on win95 gets unplayable sometimes, mostly i run it on pure dos, the perfomance difference feels like 20-40%. Never tried OS/2 before, but sounds interesting...
