CUE file is only a descriptor, which you can open in a text editor and see what file(s) it is referring to. Those will be where the music is.

You can do the finger test, find a place where the finger starts to make the problem worse (avoid the crystal and its capacitors, that will absolutely affect stuff and give a false lead).

This looks like pixel clock generation issue, likely in the analog domain. You'll probably have to carefully inspect some passives around the main chip, maybe even replace a tantalum.

The later cards with VGA use a DP or HDMI to VGA conversion chip and are subject to ~165MHz limit, so no high resolutions and/or refresh rates from those VGA ports.

You need to select the "OPL3-SA Legacy Board" and not the "Sound Controller", which is a subcomponent of the whole chip. Legacy Board should install all the different subcomponents automatically.

Because this card is not PnP, automatic detection in "add new hardware" wizard will not do much. The driver installer should have set up stuff automatically, but since it didn't you have to go to actual Add hardware and manually select INF in the installation directory of the driver package. There is no presistent storage on the card where settings get stored by SETUPSA or any other conf util. You're supposed to run SETUPSA every time on boot of the computer, although Yamaha's Win95 driver should work even without doing that. That driver package should also do any setup necessary for …

6379 is the lowest cost version, and its main issue is both channels use same VREF internally and it increases stereo crosstalk and distortion a little. 6376 and 63200 also have separate analog and digital power rails which helps to improve performance even more compared to 6379.

I found my board and it is UC4913 rev2.0, similar layout and same UMC chipset but no PLD between ISA slots or anywhere else so, unfortunately, no luck factor this time 😒 Capacitors on it are 4.7µF for the most part, but there are few 10µF ones too.
