I have tested this card on XP and 98 the driver will install fine but as soon as I try and run anything 3D the computer crashes or locks up hard. I was wondering if anything else could be tried to test it since the card does not artifact but at this point I am assuming it is beyond dead.

It could be that the BIOS is disabled, have you checked what SCSI Select reports? http://download.adaptec.com/pdfs/user_guides/aha294xuwof_ug.pdf may also help How do I even get into SCSI select? Look at the post directly above yours... Yep I realized its an open firmware card well darn.
