Sorted it! My fault... I'm using some of the REMOVED configs for my games, where each game has its own additional DOSBox config file! Once I added the joystick modifier to this, it all started working again! Thanks for the help.

As an aside, I tried Sensible Soccer. The joystick config was set to auto, and I was able to map the D-pad to the keyboard arrows. This game works fine, which makes me wonder if it is actually game related.

I'm not sure why, but when I open setup.exe it just goes blank, so I don't appear to have an option. I wonder if xpadder can work on a game by game basis within DOSBox?
