Not sure what they mean, but on the first picture the large M is actually the Matsushita (Panasonic) logo, so maybe it's something related to that? Maybe they made the PCBs and use that code as kind of a revision number as you suggest... Thanks! Not sure how Matsushita can be related to the SCB-55 …

I had similar symptom with a Voodoo 3 3000 AGP - lines were very similar to the ones you show. Only visible in 3D. Never got what was it with it as I didn't thought its worth my time - have several others of the kind and just decided to sell it to a person that is willing to work on it.

@Jager, I know its an old post but was just browsing info on the card and saw that you are looking for BIOS. I have the ELSA Winner 1000VL working and it has the same DAC and clockchip which you describe. If you still need it - I can read the PROM with TL866 and provide the file here.
