By "running regularly" do you mean running the game in Windows without DOSBox? It's possible that your sound card or drivers doesn't support MIDI. What kind of sound card do you have?

You can't run it in DOSBox. The game requires Windows. DOSBox is for DOS games.

MGS for PS1 should run fine with hardware acceleration using epsxe. As for the PC version you'll only be able to run it in software mode unless you want to buy a PCI Geforce FX.

It looks like it was fixed sometime between DOSBox 0.72 and the 6-1-2008 CVS http://www.aep-emu.de/cvscompile/dosbox.htm I just tried it in the latest CVS and it works fine.
