Run "sudo ldconfig". Normally this is run automatically when you install packages, but since this was a manual install, you need to run that command manually. What it does is make programs recognize newly installed library files.

That does not look like vanilla DOSBox. Are you trying some third-party DOSBox fork here? The missing headers are from the Munt MT-32 emulator, but official DOSBox doesn't actually have built-in MT32 emulation.

Sometimes I can't believe how good OPL can sound. Great tunes! Too bad most games treated it as a generic fallback for MIDI... The *.rad files are not compatible with the DOS version of RAD though (v1.1a from 1995.) They changed the file format.

The drive is broken, I'd say. If it can't read any floppies other than the ones it formatted itself, then you have a damaged drive in your hands.

I captured the MIDI data that is sent by ScummVM into a MIDI file. Playing that file directly in Midiplayer doesn't crash it. SCVA is not available as 32-bit anymore. Only old versions (like the demo/trial installer) are 32-bit. The modern version is 64-bit only.
