Thanks for the advice everyone. It was most likely a bad card I think. I replaced it with an Ensoniq ES1371 card and it works perfectly. The original card was a CT4810, so only had one audio output.

I can safely say my CPU speed dropped by atleast 1/3 after doing so. It's running at about the same speed as a Pentium 200MMX, even slower than a Pentium Pro. I suppose.
