Its possible the video BIOS is forcing 60Hz output in text mode for compatibility reasons with LCD displays. What does the output look like when running VGA mode 13h (320x200)? Sync polarity really doesn't matter with modern multisync monitors and its completely irrelevant to DVI-D connections.

If you ever get a chance to run it, The Matrox G400 driver for Windows 3.1x is one of the best out there. On-the-fly resolution and color depth switching via PowerDesk and it has full DCI video acceleration.

Windows/WfW 3.11 automatically disables SMARTDRV if you turn on "32-bit File Access", since that particular vxd handles disk caching in Windows..... and much faster since its a protected mode driver. Windows 3.10 lacks this option.

Some Viper VLB cards used a Weitek 5186 for the VGA core. I still haven't found any info on this chip indicating its any faster in DOS then the OTI-087. The card itself had enough problems, the worst being the broken VBE support in the BIOS that required a TSR to function correctly.

The Epiphan DVI2PCIe can capture 15khz 240p signals even though its not on the spec sheet. Technically 320x200 is really 720x400@70Hz, the actual content is line doubled.

Wake-on-LAN needs a dedicated header, the PCI bus isn't active when the machine is off.

Its doubtful a 48-bit LBA compatible version of EZ-Drive was released. Whatever is out there likely works on drives up to 137GB. Ontrack's Disk Manager might have gotten a 48-bit LBA revision though.
