If dosbox is not seeing/using the axis you want, I can suggest one alternative. Use UCR/UJR (an autohotkey script) with vJoy to map the axis you want onto a virtual joystick with the x/y axis and use that virtual joystick in dosbox.

Perhaps try the free "Hidden and Dangerous Deluxe" version it installs fine for me on win7 x64. Only problem is direct play is broken after Winxp so you cannot host but only join multiplayer.
