Could it be the mouse you are using? I know I had issues with the right mouse button on the OSX version of DOSBox using an Apple Mighty Mouse. I believe this is probably because the Mighty Mouse is still essentially a single button mouse with a touch sensitive surface.

rcblanke is correct, not sure why that file isn't present on yours after installation. I moved the disk0.id file temporarily to another directory, ran the program and got the exact same error message that you had stated in your first post.
