Are you sure with dos4gw? I thought sierra used dos4g, which is a quite different extender and dos32a is not too compatible with it... Btw: there's no need to replace the core with sb, you can always do: Copy code to clipboard 1 dos32a.exe someprogram.exe

That's what I'm using for quite some time now. I put my settings in files like *.dosbox with individual settings like cycles and autoexec and associated *.dosbox with dosbox.exe -conf "%1". Together with a custom iconhandler this is pretty nice;-)
