Normally I would think this is normal behaviour for passive matrix/DSTN screens. But you mention the symptoms are related to movement in the game... Does it have to do with contrast perhaps? The first level of Doom is darkish to the left, outdoor area to the right... Otherwise I'm out of guesses.

Is there any option to set the drive type? Like 1.44, 1.2, 2.88, etc? Otherwise try clearing/loading defaults in the BIOS.

There is the Savage4 LT. Savage4 has Windows 3.1 drivers, but they might be picky which exact chip is used.

In Awave I see instrument names, but I wonder if that data is dropped so that it generates DOS 8.3 compatible file names. It probably needs a lot of tweaking but my retro rig is in storage at the moment so I can't try it out on hardware. I wonder how it sounds. Can you do a recording perhaps?
