For best 2D/3D look for a Banshee. For best compatibility, great 2D and limited 3D look for a quality S3 Virge DX - they're very common and quite cheap. Tried exactly that, but the computer won't post. I believe setting everything to 1-2 or 2-3 causes a resource conflict witch prevents the machine from posting. J0 is the IDE led (I traced it to the IDE interface) and J3 is probably gameport on/off

Yes I have. I ran it with everything from a Matrox G200 AGP to GF 3 Ti 200 - same issue with all of them. The only card it won't BSOD with is my AGP banshee, but that's basically a PCI card in a AGP slot - still, it hangs in any 3D app (DX, Ogl or Glide)

I bet it's the Mach64 drivers... I had the same issue once with a PCI ATi card ( 3D Rage? - don't remember) + Voodoo2 -> Quake 2 worked fine in Glide, but would crash at certain resolutions in software. I installed another set of drivers for the ATi card, installed DX6.1 and the issue went away.
