It is your Voodoo3. It pulls too much power for that board revision to handle. Your card is probably fine, but you cannot use it with the FIC VA-503+ or PA-2013...at least before Rev. 2.0. FIC PA-2013 Ver 2.1 and Voodoo3 3000 AGP

Even though you've run Memtest, just to be safe I'd buy a different set of RAM and try it out. What are your memory timings set to? 100MHz CL2 is actually more demanding than 133MHz CL3 RAM.

A Lot of ISA EGA cards also had a Composite out. I don't think that the output quality will be all that good. In general, you would want to capture the VGA output.
