Looks like some (3-4) of the 'legs' of on the corner of one of the 3dfx chips has been severed. So that's likely why. Going to see if I can attempt a repair. If not, I will just eBay it. I would keep it. 8$ USD is not that bad for a (supposed) dead Diamond V2. They look nice in a cabinet and …

Have you tried putting the card into another PCI slot? Maybe it needs busmastering capabilities which it cannot access in this specific slot? Also, trying the card in another system or just under another OS should provide clarity whether its toast or not.

Screen distortion is often a sign for a damaged memory chip. GPU cores are normally very longlasting and don't tend to malfunction - and if they do, they don't produce an image alltogether.
