As it turns out the ES1868F as three possibilities for the output jumpers: speakers, line out without 3D, line out with 3D. It was set to speakers which caused the signal to be amplified way too much for my headphones. Now I have set it to line out without 3D and the output is much lower.

Sounds like poor voltage regulation on the ISA bus, probablye due to bad filtering caps on the motherboard? Try it on another motherboard and see if you can reproduce the problem. If you can't, then recap the motherboard. That makes a lot of sense and I'm considering testing with another KT7A …
