A UV EPROM is not something the board can update you need an EEPROM? Probably true. But it's a little strange as it is the original BIOS EPROM. Seems funny to me that Biostar sold motherboards which couldn't save ESCD information. :) Its not original, picture with original sticker peeled off …

Can you post a picture of the card? When i search for it on google it looks like a OPTI card not Aztech. Maybe its the IRQ for the IDE connector? See attachments for pictures. I didn't get it to work as well on a 286. But I blamed it on the motherboard not having PnP abilities. You can try another cable (40 or 80 pin cable) and play with master/slave/cs settings on the CD ROM. Options are correct but it can also be an IRQ or address conflict. Are you sure they are free?

What is the output of your config.sys and autoexec.bat. Where does it not work? Maybe try another /I: option (IRQ) /I:C or /I:A. Make sure it's free though.

I connected 3*AA batteries on the external battery connector and it seems to work fine. I used a multimeter on the positive leg when the system was turned on and it only measured 0.4V so I guess it's ok. If not please let me know.

Pin 1 and 2 on lock are for power LED.. The rest of those three headers are for keylock (that thing on the case to disable the keyboard from working. SPK stands for speaker or PC speaker.
