They are patches for the source which then has to be compiled. And note that Win9x is not supported in DOSBox. Spare yourself the pain and just use a VM instead of DOSBox.

The preferences for the CD version have a separate switch for sound and CDA music. Without exploring it I cannot say if turning off the SB sound simply gives you no sound effects or MIDI/AdLib sound effects. I would have to re-read this thread to see if NRS or tikalat said anything about Roland …
