Apparently I am not getting reliable results on the same machine... ATI OpenGL definitely has its share of quirks.

IIRC, KQ5 had manual check. At some point you need to push a boat to water, that triggers it. Not sure if it was floppy or CD version though.

Yes, I already tried that, seems to be working just fine. There is even commented out code there just to that effect. Do you have NVIDIA card to test?

The only catch is that you will not be able to print to your printer directly. Your program will print as it normally does but DosBox will output it to a .png file from which you can print to your printer. IMHO, that's better solution anyway. At least with the latest (0.8.0) version, when you start the game you get "SCUMMVM 0.8.0" on MT-32. I don't think you will get the very first message from LucasArts games which was "Lucasfilm Games" -- that was generated by the executable. Sierra used that display a lot. Legend Entertainment also …

I have fully patched XP/SP2, no problems here. It is most likely some device driver issue. Update your drivers to the latest version. In the order of likelihood to cause issues, graphics driver goes first, closely followed by sound. What do you have in your system?
