I just find it odd that some games will respect aspects and others not, regardless of dosbox's aspect= setting. There has to be a reason. aspect=true only affects game resolutions that have a vertical dimension of 200 or 400. If aspect=true, then those are corrected to 4:3. Beyond that, Dosbox …

I just tried the shareware version of Blood on my AMD Athlon XP 2400+ at core=auto, cycles=auto and it ran smooth as butter at 320x240, quite fine at 640x480 and it was even playable at 1024x768!

Just setting the fullscreen resolution to your display's native resolution (as long as its pixels are square) and setting aspect=true should do the trick.
