if you search the creative download section for es1371 you should find the drivers. I couldn't find anything earlier though, like sb16 and such.

I think the quote you are referring to was not specifically about midi. The ace model was designed to run alongside an sb with few/no conflicts. So you get gus support with less headaches. And a gus ace can play midi files.

I reckon it's a Joindata Systems board. Joindata 486 boards seem to have had names like G486EVL, G486HVL, G486SVL. The letter before the VL seems to be the initial of the chipset, eg. SVL is Symphony. That would fit with it being G486UVL indicating UMC chipset. Presumably VL indicates "vesa local" …

Have you tried cleaning the contacts on the controller board? I used switch cleaner on my lifeless SCSI VLB card today and now it seems to be working. Does your controller card have vga on it, in which case could it be conflicting with your graphics card?
