delete the mapper file and make sure it is not loadein in the dosbox console window. joysticktype ch or fcs may work. (try ch first as fcs is more complicated so move to fcs only if ch works). so first try assigning letters to each axis to check whether you can type by moving axes in the dosbox …

i have noticed (generally about the glide patch) that it cannot initialize 800x600 with real voodoo cards when you start a game with that resolution. heh, usb doesn't even work in dos so i wonder how you used it in the old days 😉 so the ps2 one is definitely worth trying (my ps2 is actually also about 104 as it has win keys), but there were no win keys in the dos days too heh.

anyway glidos your gamma setting just sux 😉 it is not increasing gamma (contrast emulation) but brightness, maybe your curve is flat? or wrong shape

sorry that was a result of using D3D textures bit depth 16-bit. Oh well, so software mode looks like 16-bit cause it is 16-bit and that is why it looks different 😀

sorry but have you checked dosbox manual for different cdrom settings? they are designed specially and only for cd audio problems
