A simple clue or advice remains. Do not tamper with the original DOS Box config and let the expert(s) advice. 😊
My 2 cents here with ALT + Enter , on how "OUTPUT" behaves with DOS and Win 3.1.
Output = Surface DOS (No) Win (No)
Output = Overlay DOS (yes) Win (No)
Output = opengl DOS (yes) Win (No)
Output = openglnb DOS (yes) Win (No)
Output = ddraw DOS (yes) Win (yes)
If this thread could be taken further as to what could be the right combination to arrive at to get the screen full , by config itself , I am game to try out. The experts can advise...
Yes I am able to play the game without issues so far, but let me get back in case of any issues...

Just to add that full screen in config ie [fullscreen=true] works perfectly without issues.
