I was sitting in the cockpit, simply turning the engines on and off. Then this happened..
Was so loud it about made me throw my headphones off my head

How about some details? Such as: what sound driver did you select for SQ3?
Considering that you commented in this thread, are you perhaps using a non-stock sound driver?

I believe I'm using mtblast.drv which I think was provided by NRS?

I don't think so, because my MTBLAST.DRV would not try to play this sound effect using PCM, as it was clearly recorded from the MT-32.
And problems with non-stock sound drivers have nothing to do with DOSBox, so they don't belong in this forum.
