I could not resist and ordered this beauty:
https://www.ebay.com/itm/276284606622
It's made by Everex but seems like only for the internal use. Cannot be found by FCC ID, seems like they didn't even bother to apply. Probably engineering sample of something like this.
Memory is presented by 16 ZIP24 pins. If it's 64Kx4 it has 512KB of memory. Year 1991.
It has 2 9 pin connectors but it seems like most of the early VGA cards and 8514/A and IBM PCG had this type of connector supporting analog signal. So one is probably for CGA/EGA analog emulation and another one is CGA/EGA.
Still have to wait for two weeks or more - there is no direct delivery to Ukraine on eBay.
Any suggestions what could it be? There were Everecs PEGA - clone of IBM PCG - it had the same VRAM chips but only one output and no daughterboards. But 1991 is quite late for that type of cards. It's pre-era of dedicated hardware accelerators - at least 8514/A. On the other hand, I see 40-pins connector on the back of the card - that could be 8086.
