If it was a 3DFX voodoo 2 2000, it would have another video port (one for the PCI graphics card, one for the screen).
Besides, the port side seems to be an APG2X/AGP4X one, not a PCI one.
I'd say it's a 3DFX Vooodoo 3 2000. Unless other Voodoo cards after the Voodoo 3 had a "2000" version.

Definately a V5 5K. Voodoo 3 never came in multichip configuration and a molex. Also those fans are a give-away 😉

Wow! I've never seen or heard of an AGP Voodoo card with a universal AGP connector like that, I had always assumed they were also electrically restricted (higher voltage requirement) to AGP 2x/4x too.
This baby would only have 16MB effective total memory with how SLI works. I'm not sure what that would do to performance in games. Makes it kinda Voodoo3-like.
That's got to be a prototype V5 5000. I didn't think they were ever released.
