Grab the DOS Tomb Raider 3dfx demo. Run setup and disable sound (just in case to avoid any mucking around) and run it.

It should just work, doesn't need any driver. Try in MS-DOS mode as well.

just to be certain have you tried a dos glide game (in real dos not dosbox and not in a win9x dos windows)

All efforts to to get a game running, even in Dos have failed.
I have to put it down to a dud card and luckily the seller is being very reasonable by offering a refund.
Heck, it happens!

Did you try to cleaning the card's pins? It could be just dirty.

Could be the caps bad.
heat up the card in advance.
No contacting QFP legs?

What's the 2D card you're using it with?

Although it's probably academic at this point, if you've returned it.

Its not the clearest pic, but a few of the pins on the TMU look goobered. Might need to get in there with a magnifying glass and a razor or needle and make sure they aren't touching.

I still have the card and forgot to mention that, in driver properties, in spite of newer drivers ie, not the ones Windows wants to load, it always says 'no drivers have been loaded or are needed for this device.................' etc etc
