The images are too bad to read the product numbers, but the card looks like a standard AWE64 card.

you can read the sticker on the third picture, but you were correct of course, it says ct4380 awe64
Yeah, it's AWE64. The double row of AWE64's proprietary memory board connector pins gave it away.
Probably a AWE64 value.
