Appears to be Gainward Power 3D II based on searching on the FCC ID printed on the card.
Here's another videocard collector who has one, if memory serves he has a VOGONS account perhaps.

Whenever you have a question about a part that you can't identify, Googling the FCC ID is usually a good place to start. There's enough people active in this hobby now where a lot of this stuff will be catalogued or at least mentioned on a website somewhere that Google can reach.

I actually wrote an article about hardware identification on the wiki yesterday 😜
It's still a stub and needs some organizing and some picture-love, but it does mention this (amongst other tips)

I could probably teach a master class in Internet-augmented computer hardware identification.
Important note that (in cases where the FCC ID is not faked, which I believe I've seen done), you can STILL pull up information regarding devices in FCC's database.
