Docs needed for old Everex card

I am rebuilding an old 5170 IBM AT and have it running, POSTing and booting from a hard disk. This AT has 512k on the motherboard, and so I bought an Everex expansion card, EV-125. This is a multi-port + memory card. I have found at stason.org and other places a basic listing of the jumpers and their possible setting, but I really need to find the OEM documentation as there is too much unknown for me to get the card working.

Anyone who has a copy of this that they can scan or point to an on-line repository, please let me know.

Thanks for your reply. Yes, I saw the manual on MZD before I posted here. My problem is partly with the I/O ports. Ideally I would disable them as I have two other cards with I/O as well and just use the memory. But I can't find anything to tell me how, or whether, that could be done.
