I believe I've found it after yet another image search various case manufacturers.
The Codegen 6061 is almost spot on from what I remember, although it or my memories aren't an exact match.

dansdata.com has a review of the case with several pics. Their about the only ones I can find of it.

I don't remember mine having the front audio ports under that bottom cover or the top fan. Mine was also a dark color with the silver plastic expansion bay covers. Dan's review only shows all black or silver and blue.
I'm only finding references to this case from Australia and Russia. Wonder if I had some knock off that made its way to North America.

Codegen cases are ultra cheap low budget cases. They used to create and manufacture new cases each year, with slightly different numbers. For what I remember, they never shipped the same model's for every continent. (though I could be wrong on that one)... As an example. I believe that the Codegen 3307, only shipped to Europe. Possible North America as well. I do not believe it was ever sold in South America, Africa or Asia.

Getting back to the Codegen part.
I believe you will have to search wide and far, in order to find that specific case, if it is indeed a codegen. Low quality, and produced in a short timespan. Not like Antec or Thermaltake cases, that were produced years on end.

Codegen used to add subtle features throughout a case's lifespan. Sometimes taking features away.
Like adding or moving front panel audio. Still keeping the same model number.
I remember that in 2005, they suddenly added a sidepanel fan-mount. (holes for mounting a fan)
It just made things a bit more interresting, building those 12 to 15 AthlonXP computers each day.
One never knew what to expect when opening a pallet of brand new Codegen cases. 🤣
