Recently picked up a 1040st. If i leave it on in GEM for a while and come back, sometimes I'll see a 3 bomb error code (address not found). but sometimes I don't.

I found an image of Defender of the Crown, and used stdisk on my DOS machine to write the ST image onto a DD floppy.

Boot up on the ST, it can read the disk. I run the .tos file, it starts to execute.. and after a few seconds exits back to the desktop.

Does anyone have any clues as to what I'm doing wrong?

You might not do anything wrong.
I would expect that the machine has some small issues (3 bombs indicates a possible memory error).
Atari ST machines are notorious for their chips getting unseated (google 'Atari Twist' and 'Atari Drop'). So one thing you could try is to open the machine and try to wiggle all chips and memory modules into place. It could just be a poor contact.
Another cause could be bad capacitors.

I think the same. Also, there were some diagnostics programs, like SysInfo, which could help finding errors.
Btw, I also recommend using TOS 1.04 aka Rainbow TOS, if possible. It has several improvements and can handle DOS-formatted floppies better. 😀
