Here are a few examples with Via Apollo 133A chipset:

Gigabyte GA-6VX7-4X

Abit VH6T (Apollo Pro 133T, supports Tualatin)
abit-vh6t-socket-pga-370-system-board-1.21__37559.1490217257.jpg

etc.

Most don't have onboard LAN but that's an easy fix with a PCI card.

In fact I will direct you to this list:

The Advance 6T has several versions, only the A6T-L version has onboard LAN AFAIK. Also QDI motherboards are pretty rare. I have a few QDI motherboards and they were a pain to find. Finding a specific revision of a rare motherboard from a relatively obscure manufacturer will be a bit like finding a needle in a haystack.
Also the A6T-L is a mini-ATX motherboard with only 3 PCI and 1 ISA slot (of which one PCI is shared with the ISA, so maximum 3 cards possible, either 2PCI+1ISA or 3 PCI at one time). No AGP.

No, I don't have this particular model.

Nope. Personally I'd find one of the larger motherboards and stick a PCI LAN card in it, but that's just me. Socket 370 motherboards with ISA slots will be in this day pretty hard to find anyway.

I think you are going to be pretty hard pressed to find a board for a good price with all those features unless you drop the onboard LAN requirement.

Here is one, but it is $75 + shipping.

Didn't see any others on eBay at the moment.

I wouldn't buy that, even at that price, as it is missing one of the points for attaching a heatsink (unless you have a heatsink that will still fit)

See my graphics card database at www.gpuzoo.com
Constantly being worked on. Feel free to message me with any corrections or details of cards you would like me to research and add.

It appears to use a SiS630 chipset http://www.motherboard.cz/mb/ecs/P6STM.htm so AFAIK no it doesn't.

Cpu cooler

Fair use/fair dealing exception

Will that motherboard allow me to use all the pci and isa slots or is the isa slot shared with a pci slot also will this cpu cooler still be able to mount on there

You know you can swap IO shields right? If you really wanted to keep the case, I would sell or swap the voodoo 3 for an AGP version, get a micro atx board with AGP and ISA and that comes with an IO shield and buy a pci network card

Then maybe you can find the rare micro atx board that does both tualatin and ISA

Unless you're into re-capping it, I'd avoid this as the ones round the cpu socket look blown to me

Why do you "need" onboard LAN?

What happens if the onboard LAN is flakey, buggy, or bad? Then you have to use an add-on NIC anyway.

And if you are planning on using it for DOS games, you really don't want onboard sound either except for a few select ones and that is going to make your search even more impossible.
