Most likely dropped from high place.

Most likely not. Left damaged CPU would be possible in theory, but only if it hit the ground exactly at that corner and did not fall too high. Right damaged CPU: no way that this can ever happen by dropping. Seems much more likely that this is both a screwdriver damage, the right one without a doubt.

How does this type of damage occur? Is it from trying to pry them out of non-ZIF sockets?

Most likely dropped from high place and landed on a corner.

or Heatsink removal with a lever

I got mine cheap(ish), knew about the missing pins, saw the damage, decided to take them anyway for... experimenting. And that's what I'm gonna do when I have some spare time.
I don't expect it to be easy (or to work at all), I imagine it's very crowded under that die cap and I also think the pads leading to the outer rows of pins are those on the lower level, making them extra-hard to reach.
And yeah, I think it's possible that contaminants will start destroying the chip once it's opened.
