I'm looking for a motherboard that meets all of the below requirements:

* S370 motherboard
* mATX form factor
* FSB: 66/100/133 MHz
* CPU: VIA C3 CPU and Tualatin P-III
* Slots: 2 PCI + 1 ISA + 1 AGP

I have looked through the List of Socket 370 motherboards and the VIA C3 Verified Motherboard List, as well as searched through specs on CNet. So far, the only chipsets I could find that meet these requirements are all from VIA:

* Apollo Pro 133T (VT82C694T + VT82C686B)
* PLE133T (VT8601T + VT82C686B)
* Apollo Pro 266T (VT8653 + VT8233)

I have a MSI MS-6368 with the PLE133T chipset that meets almost all my requirements, except that it does not have an AGP slot. Does anyone know of any motherboard that actually meets all of them? Looking for the model numbers so that I can search the usual auction sites as many of the listings do not provide enough details. If not, then I will abandon this as another pointless quest. Thanks!

DOS PC: Socket 370 / VIA C3 1.0 GHz / Voodoo2 + Trident Blade3D / SBPro + YMF744 + AWE64 + SC-7

The answer is in the list you posted, isn't it?
Chaintech CT-6VIA5T (ok, 3 PCI slots, but one is shared with ISA.)
Epox EP-3VSM (same.)
QDI Advance 10TM (P6V694T/A10TM) (exact amount of PCI slots, can't find pics if any are shared with ISA)

Ah yes, I had seen the CT-6VIA5T and the EP-3VSM on the list, but the boards I found online were either missing the ISA slot or the AGP slot. There are boards that probably have both though, so I should keep looking.

I could not confirm the existence of the QDI Advance 10TM outside of that list (and now also in Win3x.org!). Sometimes manufacturers publish specs but don't actually end up putting the board for sale. I thought it was one of those.

My experience with missing slots is that it's usually the result of an OEM ordering a custom run with the slot removed to prevent upgrades. In many cases the slot is the only part that is changed from the regular version so after adding only the slot it's completely usable in most cases. If you're somewhat handy with a soldering iron you can often make a good deal on those 'castrated' boards.
