Did this game ever come with cover or box artwork?

This is one game that I missed back in '95 when it was released and it seems odd due to the year that I'm not able to find any images of cover art or even a box that I'm assuming it was shipped in. I've seen pictures of the sequal "Rebel Moon Rising" but nothing for the first game.

Rebel Moon is a first-person shooter bundled exclusively with the Creative Labs 3D Blaster PCI or VLB 2D/3D accelerators. The game is only compatible with a video card utilizing the Rendition Verite 1000 Chipset.

I guess that would explain it. It's still playable via vga though so the later part isn't correct.
