A few years ago i posted this report.

Re: dgVoodoo 2 for DirectX 11

With the latest WIP the game boots but there are two problems. Videos wont show up (Cinepak compressed videos) and most 2D graphics are corrupted, some of them are invisible, some sprites are incorrectly placed (flipped parts), etc. The 3D floating head is also invisible, maybe it is not a 3D characters but a render-to-texture thing.

I've made a comparison between DXWnd and dgVoodoo latest WIP. DXWnd has his own problems, like that blinking background, but i havent messed up with the configuration so it could be fixed (maybe).

Ok, finally I managed to get this to work.
It has an 16 bit installer so I had to install it through a VM.
After that, I ran HXConfig, saved the configuration and added InstallDrive key manually among the other config keys (this is important otherwise I just got a blank screen in the game).

I copied the dgVoodoo files into the game folder, and also, d3drm.dll was needed so I copied that too.
Movies are rendered though GDI. I didn't check it out yet what it does exactly but I guess I need to improve GDI hooking in dgVoodoo.

Other than that however, the game just worked fine for me, I only encountered one glitch on the sky at a certain point (see the video). Actually, the game has a cool retro feeling in high res with dithering:

Are you using the latest version of d3drm.dll (5.1.2600.0, 350208 bytes in size)?
