Hi,

what is name of this Sound + modem combo ISA card?

Photo is in low resolution. Main chip looks like OPTi 929 or 930. Codec is Crystal CS4231 od CS4248.
Looks full-lenght similar like PhoneBlaster from Creative.
Wih MCD+IDE and older OPTi chip is from 1994 od 1995 year.

Didn't find it on th99, though there was very few sound cards picced there.

There were quite a few similar cards shipped with systems around 1995. One I knew of that had a CS4231, OPTi controller IC, OPL3 and modem functionality was from Boca Research. Try to look up model SE1440 to see if that's similar.
