For Win3.1 I used MOD4WIN. You can use WinAMP or UN4Seen for newer Windows.

For Amiga take a look here:

I still use ModPlug on my PC with Windows10 on it. Seems to work just fine and I've always liked how the module files I have in my library sound through it.

Can't answer about the best Midiplay for win3x though. I'm pretty sure I was just using the built in media player back in the day with my wave table boards in my setup back then.

For modern Windows I use foobar2000 (https://www.foobar2000.org/) for all my audio files; there are plugins for just about every format you can imagine, and its interface is fairly customizable. It's been my go-to player since the early 2000s, when Winamp started getting a bit crappy.

I don't recall any MOD players for Windows 3, IIRC everything was still done under DOS until Win 9x. If they're out there, I never saw them.

For just playback, I was quite fond of Cubic Player (OpenCP, it's included with FreeDOS), or the tracker appropriate to the format.

And, considering the MOD format originated on the Amiga, there should be software for it, but I was never into Amigas, so I don't know much outside of the history of it.

As mentioned above, the MOD format originated on the Amiga, so there are literally hundreds of MOD players for that platform: http://aminet.net/mus/play

Delitracker, Eagle Player and Hippo Player were probably the most popular.
