Attached is an ad from Computer Gaming World (January 1993 ) advertising a sound card from MIDI Land titled the "Okey Dokey Sound Card".

Near as I can find, this is the only reference to this particular sound card. It's ad by Midi Land, Inc. Weirdly the "Okey Dokey Sound Card" is even listed with a TM symbol suggesting the name was trademarked.

Did this card ever actually exist? Was it vaporware? Does the pictured card still exist somewhere even if just in prototype form?

Can't comment on the release or existence in real life of the card, but the picture shows what looks like a pretty typical Sound Blaster (1) clone, with OPL2, 8051 microcontroller, non-standard DE9 joystick/midi port, volume knob, 3.5mm jack for something and USP those two RCA connectors instead of 3.5mm jack(s).

I think the same.

I also believe to see an NE558 timer IC and the required glue logic.
These ~5 TTL ICs on the left.. My Lindy gameport card has them, too.

Edit: Here it is (hi-res). Please have a look at these wonderful discreet parts!
Even the little charging caps for the 558 timer's time base are visible. ^^
