These were a real mess of same looking cards with many model numbers and driver variants. There was a recent thread here gathering some more of them, with the results uploaded to Vogons Drivers. Try everything in the Creative section with descriptions for Audio PCI, PCI 128, etc etc.

Indeed, these drivers work with my CT4700 in Windows 98 MS-DOS mode.

1) ES1370/CT5507, VEN_1274&DEV_5000

I have to say that Joe & Mac Caveman Ninja sounds like ADLIB with these drivers.
