It looks like the top connector has a lot of signals from the ISA side connected to it, but what is it really for I wouldn't know...

Looks like ISA pass through, perhaps for a Daughtercard, its not a creative card either, perhaps its some proprietary card.

I have seen similar weird sound cards based on the vibra16s.
For example this thing:

It is for a siemens nixdorf FD210 All in one system.

Judging from the strange bracket on your card, it seems it came from a special application / integrated system.
