So I've read that the PCX2 doesn't play nicely with Nvidia cards. Unfortunately, the only non-Nvidia card I have is a Rendition V2200 which isn't supported by the PCX2. I have attached an image of Quake II using the PowerSGL renderer and I'm pretty sure it shouldn't look like that! Could this be caused by the Nvidia issue or is it just broken?

PowerVR PCX2 cards' incompatibility with nVidia is only regarding the Direct3D HAL, they're not conflicting when it's about SGL (and the MiniGLs towards that) so it shouldn't be causing that. Try fullscreen and 16bpp

OK its broken for sure, I tried my other PCX2 and it works just fine in this system. Before testing with the other card, I tested fullscreen 16bit which didn't help. Any ideas what could cause such an issue and how it can be repaired?
