Here is what I did:
Hardware:
1. Put in Voodoo2 in PCI slot
2. Install drivers when prompted (Diamond Monster 3D II V4.10.01.0207)
3. Reboot
The game:
1. Install the game (I have the original UK release). I used the default installation path (C:/S2)
2. Run Setup.exe to set sound (SB16 Stereo - 220, IRQ5, DMA 1)
3. Copy the patch files (s23dfx.exe & s2v2.bat) from the rarfile in first thread to the game directory (C:/S2).
4. Run s2v2.bat
Game runs in both Windows98SE DOS Window and in pure DOS (F8 at boot).

Standard autoexec.bat and config.sys with HIMEM, EMM386, CD-ROM, mouse and soundblaster settings.
Do you see the 3DFX logo spinning? The black screen may take some time to get past on certain CD-ROMs/computers. My Compaq for some reason used much more time to load the game than my other machines. So have you waited a minute to see if it does actually start up?
