I have not tried it but since the user name states 5x version I bet it works.
Univbe 5.2 for DOS is a weird beast - all I could find was a version with a German installer - programs are in English so no big deal I guess.
If anyone has the English DOS version please upload it

I believe I tried that already. The files extracted, but it also says that it is not configured properly and to run the installation. So, not sure what I am doing wrong.

Here's the BIOS v2.09 dumped from my card with NSSI as suggested by keropi.

EDIT: added the DOS utilities package from the drivers CD, the rest of the stuff in the CD is just Windows 3.1/95/NT drivers, OS/2 drivers and DirectX + MS Internet Explorer.

Are you able to provide the full CD which came with this card? You might be able to break it up into pieces if it is too large.

Brief update... Figured out Univbe 5.2. Univbe 5.3 and Univbe 5.2 use the same registration file, IO.IDX in C:\ , and for some reason Univbe 5.2 installation won't override 5.3's IO.IDX file. You need to delete the 5.3's hidden io.idx file, then install Univbe 5.2. To use univbe 5.3 again, you need to put the old io.idx file back in c:\. The only benefit I could see from 5.2 over 5.3 was that it uses 12 KB of conventional memory as opposed to 13 KB. Both versions have VESA 2.0 support.

Second update... I was able to use TheMobRules's VGA v2.09 BIOS from his Diamond Stealth 64 DRAM T VLB card. Seems to work and supports VESA Modus 100 640x400, which my BIOS v2.01 did not. The draw back is that I can no longer soft-reset the system, e.g. by ctrl-alt-del, or by software controlled reset. The system reboots to a blank screen in such a case. So, back to using v2.01 BIOS and hoping that someone with V2.02 will surface.

640x480x256 and higher modes would not work on my similar card, a Diamond Stealth 64 2MB VRAM PCI. It uses the S3 Vision964 chipset. Bios version is v1.05H.
S3VBE20 loaded but didn't do anything, still no 640x480 in Quake. I tested all the UniVBE versions from 5 onwards and found 6.7 the best (it also requires no registration).

Benchmarks
Without UniVBE (Card has a default VBE 1.2 bios)

Quake = No option for 640x480 (No game at 640x480x256 or > runs)
Vspeed = 19.7 MB/s (BANKED)

With UniVBE 6.7

Quake = 20FPS 640x480
Quake = 41FPS 640x480 + FASTVID
Vspeed = 46.82 MB (BANKED)
Vspeed = 76.68 MB (BANKED) + FASTVID

Vspeed did not give LFB results, don't know why. I confirmed LFB is working though as enabling it in Fastvid doubled the frame rate in Quake.

UniVBE 6.7 was realeased August 2002. I also have UniVBE 7.2 for DOS but strangely it has an earlier date of Feb 2002 . It gives the same results as 6.7.
The speed increase with UniVBE 6.7 alone is over 100%. With FastVId as well this card performs as fast as my Geforce 4200ti in Quake. Benchmarks performed in DOS 6.22 on a PIII@500 on intel 440BX board.
