I'm experiencing this issue and I had also in the past using the phil's benchmark pack the test at 640 fails with this gpu.
That's happening on my super socket 7 motherboards (both via and ali).
I guess it's a problem of this generation of video cards because if I use the Geforce 4 the test runs fine, while with geforce 2 the screen goes black and system hang up.

Any idea if there is a workaround?

Edit: I also tried the version 1.04 without any results

Edit2: Issue resolved, I switched from DOS4GW to DOS/32 Advanced DOS Extender now I can run the benchmark at 640 without any issue
