I have a Gigabyte GA-612 (Intel i740) that I wanted to upgrade the video BIOS of, as it is reporting a version of 220 in the display control panel.

I found the vflash.exe that came with the latest BIOS release 239 to be troublesome, as it wouldn't detect the flash chip on my card. However, the vflash utility from an older release was able to successfully detect and flash the latest BIOS.

The vflash that did not work is 76,300 bytes and is dated 9-23-98. The one that did work is 42,812 bytes and is dated 7-24-98.
