The question is simple....
If you can connect a roland MT32 on sound blaster joystick port, why some people would buy a MPU401 card?

The answer is very simple, too.

For correct MIDI device handling, be it UART mode or INTELLIGENT mode.
SB and other cards' midi out are often flawed and mostly UART only. What is worse, certain games will only work with a real MPU 401 interface card (Laser Squad comes to mind first but many Sierra etc titles need patching if you attach your MIDI device to a Sound Blaster)

There is also the hanging note bug to deal with. Using most Sound Blasters for digital audio and MIDI causes the MIDI device to hang on some notes, sometimes requiring a reset to clear the stuck note. This happens regardless of game, its a bug in the sound blaster's chipset.
Using a seperate MPU-401, or indeed, a different brand of sound card, gets around this problem.
