I recently aquired this video card, is not working and I'm trying to figure out where it comes from:

under the sticker it gets even more curious:

Any idea of where it comes from?

You may want to double check those image links/embeds as they are all broken for me. As for the card, I'm just as puzzled as you are.

Vendor Sierra Semiconductor (then PMC-Sierra, lastly Microsemi): seems like Sierra Swift II 1MB ISA, unfortunately no more info about this.

Sierra is well known for their RAMDACs.
I thought they only had one graphics chip - SC15064 a.k.a. Falcon/64, found on some PCI cards.
But no, obviously they also had two chips for ISA - https://www.electronicproducts.com/graphics-a … -24-bit-ramdac/

Nie tylko, jak widzicie, w tym trudność, że nie zdołacie wejść na moją górę, lecz i w tym, że ja do was cały zejść nie mogę, gdyż schodząc, gubię po drodze to, co miałem donieść.

oh,but this is the sc15012, not sc15010, I wonder if is just a silicon revision or a different product, perhaps a prototype?

The difference is clearly stated in the linked article:
"Two versions of the chip work with dynamic RAM (SC15010) or video RAM (SC15012) frame buffers"

Well, not fully clearly...
Your card is SC15012, but the memory chips are 44C256, ie. DRAM.
I guess SC15010 only supports DRAM, and SC15012 supports both DRAM and dual-ported VRAM.
